Clinical Health Network for Transformation (CHN) started as a collaboration amongst Planned Parenthood affiliates in 2017 as a way to combine the structure of a managed services organization with a clinically integrated network.

Our goal is for patients and staff navigating a post-Roe world to benefit from standardization of processes and workflows, enhanced quality, and robust reporting. We work to position member providers to:

• Expand access

• Improve care delivery

• Achieve better outcomes for patients

• Attain more favorable reimbursement

• Collectively execute innovative initiatives that transform how care is provided to patients

CHN provides services to 15 affiliates across 19 states. With its integrated services, we are able to connect patients to the care that they need – even if that is across state lines.
